Please note, filling out this form does not guarantee a spot with our practice. We encourage those with immediate need to continue to seek services elsewhere. We do our best to accommodate those on this list based off of clinical appropriateness (area of practice) and availability. We recognize the need that currently exists within the field of mental health and maintain a wait list with the aim of providing an option for those that feel discouraged in their search for services. We will reach out via email to see if the potential new client is still in need of services when an opening with an appropriate therapist becomes available.
